After School Programs (ASP)

  • Complete the ASP Appointment form to schedule a time for program staff to register your family.
  • Submit your ASP Program Fee here

Bring the following documents to your child’s registration appointment:

  • Child’s Birth Certificate
  • Child’s Immunization Card
  • Monthly income statement/Check Stub
  • Proof of Food Stamps/Government Cash Aid
  • Proof of Address
  • Parent’s Vaccines- MMR/TDAP/TB Test/Influenza
  • Birth Certificate(s) of all children under 18 years, living in the household
